To whom it my concern:
My name is Kenneth Ray Rogers Jr. and I am under the care of Premier Pain Solutions located at 5710 Oleander Dr Ste 201 Wilmington, NC 28403. Phone: (833) 365-7246. I would like to say that I am under the care of a particular physician, but this is not the case. For the past year I have had a virtual “Merry-Go-Round” of physicians including Dr. McLaren, Dr. Kathy Lewis, Dr. Randy Kimery, PA Ortiz-Ruiz, and Dr. Batch.
The overall reason why I am seeking pain management services is that I am suffering from tremendous pain caused by the following conditions of the spine: MODERATE CENTRAL CANAL STENOSIS FROM L3 TO S1.
SEVERE BILATERAL NEURAL FORAMINAL STENOSIS AT L5-S1.
After many medical consultations, it has been determined that surgery is the pathway for some relief of extreme pain caused by my conditions, however, before surgery can take place my insurance requires me to do the following: Obtain imaging of spine, seek medications for treatment, physical therapy, and an epidural. I am currently in progress of achieving these requirements.
I seek medication prescriptions from Premier Pain Solutions to relieve pain and suffering until surgery. This service was denied for me on 29 July 2025, because the physicians claim that my pain patch Butrans 10 Mcg/hr did not “populate” a Urine Screen requested by Kathy Lewis on 28 May 2025. I was made aware of this during my visit with Jesenia Ortiz-Ruiz the following month. Jesenia requested me to take another Urine Screen to test if the Butrans Patch would “populate” the screen. A day later I received a phone call from a manager at Premier Pain Solutions located in Wilmington, that gave me instructions to come in that day by 3pm for a “random urine screen”, because the previous screen conducted a day before was destroyed on the advice of Kathy Lewis without being tested. The manager wanted to be sure that a Butrans Patch, from the new prescription provided by Jesenia Ortiz-Ruiz, would be located on my arm during this screen. I informed the manager on the phone, that I have not obtained the new prescription, and that said prescription would not be available until 3 July. The manager later called me the same day and stated a new Urine Screen was not necessary until my visit with Randy Kimrey on 29 July.
On 29 July 2025, I was informed by Randy Kimery that there were multiple Urine Screens in which the Butrans Patch did not “populate”, I was alarmed by this revelation! I wear the Butrans Patch as directed by the written prescription, flawlessly. This medication is my only lifeline away from catastrophic pain, pain that has resulted in at least two Novant Health/New Hanover Region Medical Center Emergency Room visits! I am reporting “to whom it may concern” that I can provide pictures of my arm, in which any person with normal vision, can easily make out the impression the Butrans Patches leave behind, especially visible with the contrast of my sun-soaked skin around them!
Additionally, at no time have I ever used or would even consider using, the Butrans Patch for anything else other than providing some relief from catastrophic pain, pain that has completely RUINED my life over the past year!
Furthermore, there are many credible reports that the Butrans Patch tend to give off false negatives, and that relying on Urine Screens alone should not be conducted for the evaluation of pain management. The management/physicians at Premier Pain Solutions must not be aware of these reports, or choose to ignore them, which jeopardizes my healthcare needs.
In summary, I have addressed all these issues with my physicians/management at Premier Pain Solutions located in Wilmington, and they have either made accusations that I have been misusing the medication, or conveying Drug Enforcement Agency requirements, that have ABSOLUTELY NOTHING to do with my healthcare. Now, I must seek another pain management service, further delaying much needed pain medication, which will no doubt result in another catastrophic pain visit to the Emergency Room of Novant Health/New Hanover Regional Medical Center!
